Emergency medical forms signed by the parents must be
on file in the school office on the student’s first day of attendance.
It is the parent’s responsibility to keep the school informed
so that names, addresses and phone numbers on the child’s application
and medical forms are kept up to date. City Tree holds a Student Accident
Medical Policy for each child. Parents must accept full financial responsibility
for the deductible amount or services not covered by our policy. First
aid supplies are kept at school for simple cuts and abrasions.
